Psilocybin trial delivers striking PTSD remission rates
A small United States pilot trial has reported that nine of twelve veterans with treatment-resistant PTSD were in remission a month after psilocybin therapy.

Results from a small American pilot trial have renewed attention on psychedelic-assisted treatment for post-traumatic stress disorder. Researchers at Ohio State University’s Center for Psychedelic Drug Research and Education gave twelve military veterans with severe, treatment-resistant PTSD two doses of synthetic psilocybin, 15 milligrams followed by 25 milligrams, spaced two to three weeks apart and wrapped in roughly sixteen hours of psychotherapy. One month after the second dose, nine of the twelve no longer met the criteria for PTSD, and clinician-rated symptom scores had fallen by an average of 27.5 points (1).
Entry to the trial was tightly restricted. Participants had to be aged between 21 and 64, carry a DSM-5 diagnosis for at least six months, and score 35 or higher on the Clinician-Administered PTSD Scale. Treatment resistance was defined as at least three months of SSRI or SNRI medication plus six months of psychotherapy with symptoms still present (1). Demand far outstripped places: more than 3,600 people began the online prescreening for a study originally designed to enrol fifteen (2).
One finding may prove as significant as the headline numbers. Symptom scores began dropping during the preparation therapy, before any drug was administered, and those who improved most beforehand did best at the one-month mark. Senior author Alan Davis described the therapy rather than the compound as the site of change, saying “That’s where the magic actually happens.” (2)
The results land amid rapidly shifting policy in the United States. In July, the Departments of Veterans Affairs and Health and Human Services signed a memorandum of understanding to expand psychedelic research, train clinicians to administer such medications should they win federal approval, and gather data for regulators. VA researchers are already involved in more than twenty trials examining substances including MDMA and psilocybin (3).
The authors are careful about what a twelve-person, open-label study can show. There was no control group, follow-up ran only one month, and small uncontrolled trials typically produce larger effect sizes than randomised ones. Larger controlled trials are needed before efficacy is established (1). Meanwhile, less headline-grabbing work continues: VA Boston researchers have identified blood biomarkers linked to blood-brain barrier integrity that may predict PTSD symptom severity and memory problems five years ahead of time (4).
